Well the black screen issue is quite wide spread. On the Callofduty.com forums, the one of many threads on there about it has 16 pages of complaints about it and still no solution.

Was doing the same for me, although I'm on XP not Vista. Fixed the problem by pressing Ctrl+Alt+Esc and there was a popup from my firewall asking if I wanted to unblock the game.
